Student care and conduct records held by the University of Louisville Dean of Students Office are protected by FERPA (Family Educational Rights & Privacy Act of 1974). As such, the Dean of Students Office is limited in the scope of information we can share with individuals other than the student. However, there may be times when a student wishes to grant another party (such as a parent, family member, attorney, future employer, other institution, etc.) access to their care or conduct records in compliance with University policy.
Students who are currently enrolled at UofL who wish to share care or conduct records with another party may complete this DOS FERPA Release Form. This release form only applies to the information and records held by the UofL Dean of Students Office. Permission may be revoked in writing by a student at any time. This form requires students to log in with their UofL username and password.
Individuals who were students at UofL, but are not currently enrolled in classes and therefore cannot log in with their UofL username and password, may contact the Dean of Students Office at (502) 852-5787 or dos@louisville.edu for an alternative method to grant permission.
