Report concerns about a University of Louisville student’s behavior, such as threats, mood changes, or isolation, for a follow-up on their well-being.

Conduct Report

If you believe a student has violated the Code of Student Conduct, use this form to report potential non-academic violations or concerns.

Compassionate Withdrawal Inquiry

Iinquire about a late or post-semester class withdrawal due to extenuating circumstances. Follow the instructions emailed to you after submission.
