If you have been hospitalized, the Dean of Students Office (DOS) is here to help!

Academic & Campus Resources
We encourage you to follow up with your instructors and copy DOS on your outreach.
Considering the time of the semester, coursework completed, and grades, you may want to explore options such as an incomplete, compassionate withdrawal, pass/fail, half-term classes, etc. with DOS staff.
If your hospitalization was related to a disability, you may be eligible for ADA accommodations through the Disability Resource Center.
A UofL handicap permit is required to park in accessible parking on campus. Contact the Parking Office to obtain a permit if needed.
If your hospitalization was related to pregnancy, your absence falls under Title IX & UofL’s Pregnancy Policy. Please work with DOS to learn more and to notify your instructors.
UofL has a variety of resources to help you with your academics. Consider utilizing the following resources to help you stay on track.
If you need to withdraw from your courses, fill out the Compassionate Withdrawal Inquiry form.
A DOS staff member will respond and assist you with the withdrawal, tuition petition, scholarship leave of absence, etc.
Financial Information & Resources
Have you received bills from the hospital/EMS? Before paying, review your bills using this USNWR guide, which can potentially save time and money.
The UofL Insurance Advocate can assist students with student insurance, Primary Care Health Fee, insurance and health fee waivers, insurance questions, and claims/billing issues. You can also visit CMS.gov for more insurance guidance and support.
Most hospitals provide financial assistance. Visit the following links to learn more:

- If you were in another facility, google the name of the hospital with the term financial assistance to locate information assistance.
If you are having issues paying for medical bills, here are a few resources that may be able to help:

- KY Prescription Assistance Program
- Check with the manufacturer for coupons and/or patient assistance.
DOS Care & Support
Call (502) 852-5787, email dos@louisville.edu, or visit the office in-person in the Student Activities Center W301.
DOS will follow up after your initial meeting to see how you are doing and to follow up on:
- Your classes and if instructors are working with you to catch up or discuss other options.
- Check-in on follow-up appointments and referrals and provide additional support services as needed.
- Discuss financial resources and options if you are struggling financially.
- Talk about safety planning, self-care, and other strategies and coping mechanisms.
Healthcare Follow Up
Review your discharge paperwork and follow up on any necessary action items and patient instructions.
Schedule and attend any follow-up appointments with physical and/or mental health providers.
Need to find a medical provider?
- Call your insurance for a list of providers near you.
- No insurance? Contact a Community Health Center.
- DOS can assist with scheduling a triage appointment with a clinical case manager in the Counseling Center or a psychiatric nurse practitioner in Campus Health.
Support Systems
Your RA and Hall Director can be a huge support as they have specific training and are knowledgeable about resources. If you need housing-related accommodations related to a disability, contact the Disability Resource Center.
If they are aware of your situation, how can they help support you?
Is your family aware of your hospitalization? Do you want to involve them in the decision-making about school? If yes, ask DOS about a FERPA release.
What other supports do you think might be helpful to you at this time? Faculty? Roommates? Classmates? Etc.

If you need to speak with a counselor ASAP, call (502) 852-6585 or visit ULCC (SAC W204) for an Urgent Consultation, M-F 9am - 4pm. Students can speak with a licensed counselor 24/7 by calling the ULCC number.
CHS provides free psychiatric care for students and can provide physical care (which can be billed to insurance). Call (502) 852-6479 or visit the CHS website for more information.
ConcernCenter is a database of all UofL resources. You can search by keyword, general needs/questions, or specific departments.
