About this petition

Use this form to request exceptions to standard academic requirements, including graduation hours, 300+ level hours, GPA, course load limits, residency, readmission from suspension, course substitutions, academic forgiveness, and other requests such as Fresh Start. All petitions are reviewed by the A&S Admissions and Appeals Committee, a faculty committee that will consider your request and notify you of a decision via email.

If you would like a paper copy of the curriculum petition to submit in person with your documentation, email asappeal@louisville.edu. Completed forms can be turned in at the front desk of the A&S Advising Office on the 1st floor of Gardiner Hall.
