Check Your Eligibility
Meet these basic requirements to join:
Citizenship and Age:
- U.S. citizenship (or ability to obtain before commissioning)
- At least 17 years old with parent/guardian consent
- Under 31 years old at commissioning
Academic and Physical Standards:
- Maintain good academic standing
- Meet Air Force height and weight standards
- Pass Physical Fitness Test (pushups, sit-ups, 1.5-mile run)
- Pass DoD Medical Evaluation

Contact our cadre to help you register for Aerospace Studies courses. Our experienced team will guide you through course selection and scheduling.

Create an account in WINGS
Visit the WINGS website and click "Apply for AFROTC" to create your account. After creating your account, complete the "My AFROTC Application" within your WINGS profile.
Follow our step-by-step guide for detailed instructions. Pay special attention to the "Releases and Forms" section.

Pre-Participatory Sports Physical (AFROTC Form 28)
Take this form to a health clinic or doctor for a sports physical. Medical staff instructions are included on the form. After completion, bring it to our office for cadre signature.
Note: They do not need to do a body fat measurement

Record of Emergency Data (DD Form 93)
Complete both sides of this two-part document with your personal and emergency contact information. Leave blocks 9-10 and 11-14 blank.
Our cadre will serve as witness for your signature.

Privacy Act Statement - Healthcare Records (DD Form 2005)
Read and sign this healthcare records statement. Include your signature, Social Security number and date in blocks 5-7.

Drug Testing Policy (AF Form 2030)
Read, understand and sign this policy document. Parent/guardian signature required if you are under 18. Print and bring to the AFROTC Office for cadre signature.
Important: This form must be signed by AFROTC Cadre.

Mail Access Authorization Release (for official US Air Force Correspondence)
This optional form authorizes official U.S. Air Force correspondence. If you choose to sign, parent/guardian signature required if under 18. Print and bring to our office.
If you choose not to sign the form, please inform the AFROTC Cadre.

Two memorandums: one from AFROTC requesting student access, one from student to University of Louisville. Parent/guardian signature required if under 18. Print and bring to our office.

- Social Security Card
- Naturalization Paperwork (if applicable)

Create a DoDMETS Account (To begin the medical indoctrination process)
Complete your medical processing by setting up your Department of Defense Medical Examination Test System account:
- Visit our AFROTC office with your required documentation
- Check email for DoDMETS access information
- If no email received, visit DoDMETS.com
- Login with your SSN (username) and last name (password)
- Schedule medical and optometry appointments (no cost to you)
Pilot Track: Test of Basic Aviation Skills
Interested in earning your wings? All potential Air Force pilots must complete the Test of Basic Aviation Skills (TBAS).
To Schedule Your TBAS:
- Download the TBAS Candidate Form
- Complete the form
- Bring it to Room 102 Dougherty Hall
